Passport Acceptance Agency

To apply for a passport, passport card or renew an expired passport please call for an appointment. You need to apply in person

• If you are applying for the first time
• If expired passport is not in your possession
• If your previous U.S. passport has expired and was issued more than 15 years ago.
• If your previous U.S. passport was issued when you were under 16.
• If your current valid U. S. passport has been lost or stolen.

What you will need to acquire a new passport is as follows:

  • Original certified copy of your birth certificate with raised seal (the applicants name and both parents names MUST be on the original certified copy of your birth certificate) or original naturalization papers, or an expired passport that was issued after you turned 16. No copies can be accepted. These original documents must accompany your application and will be returned to you by the State Department.
  • Valid personal identification such as a driver license.
  • A completed and unsigned application. All applications must be signed in the presence of an authorized passport agent.
  • Children that are 16 & 17 years of age are required to have a valid State Drivers License and/or valid State Issued I.D., as well as one parent present with them, while children UNDER the age of 16 are required to have both parents present with them at the time of applying for a U.S. Passport. Children who are 16 & 17 years of age who DO NOT have either a valid State Drivers License and/or valid State Issued I.D. MUST have both parents present when applying for a U.S. Passport.
    If one parent is unable to appear in person with a child UNDER the age of 16, the absent parent must fill out and notarize a Statement of Consent form, along with a front and back copy of the absent parents Valid Drivers License and/or Valid State Issued I.D.
    If the child legally only has one parent, then the following must be provided at the time of application:

    • Court order granting sole legal custody to the applying parent (unless child’s travel is restricted by that order).
    •  A court order specifically permitting the applying parent to apply for the child’s passport.
    • Judicial declaration of incompetence of the non-applying parent.
    • Death certificate of the non-applying parent
    • Photocopies and notarized copies are unacceptable.
    • One passport photograph. Size should be 2” X 2”. White background.

Fees for a new passport

  • $130.00 check or money order payable to the U.S. Department of State.
  • $35.00 check, cash or money order payable to York Township.
  • A passport card would be an additional $30 to the U.S. Department of State

Fees for a new passport for a minor (under 16)

  • $100.00 check or money order payable to the U.S. Department of State.
  • $35.00 check, cash or money order payable to York Township.

A passport card would be an additional $15 to the U.S. Department of State.

All fees are non-refundable.  Payments are to be made by check (personal, certified or travelers).  No cash or credit/debit cards will be accepted.

If born in Illinois you can go in person to the County Clerk’s office of the County in which you were born and purchase a Birth Certificate. You will need to request the Long Form with your parent’s information included on the certificate.

Passport Renewal

If you can answer yes to all of the following questions you can renew your expired passport on your own by mail.

  • You possess and can submit your most recent passport.
  • Your current passport was issued less than 15 years ago.
  • You were at least 16 years of age when your most recent passport was issued.
  • You are using the same name as your most recent passport or have a marriage license or court order that you can submit as proper documentation to reflect the name change.

If you were able to answer Yes to each question please send the following to the National Passport Processing Center:

  • Your expired passport.
  • A new passport photo.
  • Check or money order for $110 made payable to the U.S. Department of State (6-8 weeks for delivery).
  • Documentation to reflect a name change.
  • The completed form DS-82 Application for Renewal.

The Mailing address is found on the instruction page of the DS-82 application.  If sending to Expedited address please make check or money order for $170 (2-3 weeks for delivery).

Passports take up to six to eight weeks to receive and are valid for 10 years (5 years for minors).
Expedited Service is available for an additional fee and guarantees delivery in two to three weeks. If you are traveling in less than 14 days you will need to apply at the Chicago office. Please make an appointment with:
