Passport Services
To apply for a passport book, passport card, or to renew an expired passport, please call to schedule an appointment. Applications must be completed in person.

You must apply in person if:
- You are applying for a passport for the first time
- Your expired passport is not in your possession
- Your previous U.S. passport expired more than 15 years ago
- Your previous U.S. passport was issued when you were under age 16
- Your current valid U.S. passport has been lost or stolen
Documents Required
To apply for a new passport, you must bring:
Proof of Citizenship (one of the following):
- An original certified birth certificate with a raised seal
- The applicant’s name and both parents’ names must appear on the certificate
- Original naturalization papers
- An expired U.S. passport issued after age 16
Photocopies are not accepted.
Original documents must be submitted with your application and will be returned by the U.S. State Department.
Identification
- A valid driver’s license or state-issued ID
Passport Application
- A completed but unsigned application
- Applications must be signed in the presence of an authorized passport agent

- One 2” x 2” passport photograph
- White background
Requirements for Children
Applicants age 16–17
- Must have a valid state driver’s license or state-issued ID
- One parent must be present
If the applicant does not have a valid driver’s license or state-issued ID, both parents must be present.
Applicants under age 16
- Both parents must be present at the time of application.
If one parent cannot appear in person, the absent parent must provide:
- A notarized Statement of Consent form
- A copy of the front and back of their valid driver’s license or state-issued ID
If a Child Has Only One Legal Parent
One of the following documents must be provided:
- Court order granting sole legal custody to the applying parent (unless travel is restricted)
- Court order specifically permitting the parent to apply for the child’s passport
- Judicial declaration of incompetence of the non-applying parent
- Death certificate of the non-applying parent
Photocopies and notarized copies of these documents are not accepted.
